Case Management

Upon admission, each patient is assigned a Case Manager (Occupational Therapist or Social Worker). Patients work individually with their Case Manager throughout their admission. Regular meetings with patients help move patients through the goal-setting and Patients are asked to consider areas of their life that have been negatively affected by their pain, which they hope to either improve or resume involvement in. The Case Manager then works with patients to enable utilization of strategies and techniques taught in the Program to develop more adaptive behaviours in order to achieve these goals. Case Managers frequently liaise with various community partners involved in their patient's circle of care. |
