Palliative Care Consultation Teams
A Palliative Care Consultation Team is available to inpatients at both the Hamilton General and Juravinski Hospitals. Patients are referred by their attending hospital doctor.
What is Palliative Care?:
Palliative Care aims to improve a person's quality of life through improved symptom management while addressing psychosocial, emotional and spiritual issues. It is care that is appropriate at all stages of a person's illness.
Our Service:
Interdisciplinary teams including a doctor, nurse and social worker provide care for adult inpatients and their families who require the specialized skills of a palliative care service. Our professionals work with the patient, the family and other members of the health care team.
Our team will work with you to:

Relieve suffering, including pain and other distressing physical and emotional symptoms

Assist with future care planning

Help with discharge planning
