We offer educational interdisciplinary groups for OHIP patients. Collaboratively with our patients and referral sources, our goal is to enable adaptive changes in chronic pain sufferers so they can improve overall functioning and quality of life. The interdisciplinary team includes Physicians, Occupational Therapists, Pharmacists, Nurses, Physiotherapists, Social Workers, Psychologists and Psychotherapists. Upon receipt of referral your patient will be booked for an assessment by a member of our Allied Health Team. Patients will be provided with a menu of structured education, fitness, relaxation and goal setting opportunities based on recommendations from the Team. A treatment plan will be provided to the patients and a copy will be faxed to their Family Physician.
We continue to provide interventional and medical management for chronic non-malignant pain conditions. This includes epidural steroid injections (at three levels: cervical, thoracic and lumbar), facet joint injections; radiofrequency lesioning, stellite ganglioin and lumbar sympathetic blocks, acupuncture, paravertebral injections and a variety of minor nerve blocks.
A faxed referral form from the physician is required for all OHIP services.
