Endocrine Clinic

Our team provides family-centered care to infants, children, and youth up to 18 years of age with a variety of hormonal conditions including growth, puberty, thyroid, parathyroid, adrenal, pituitary, menstrual irregularities, disorders of sexual development, Transgender Health, and bone health conditions.

Patients/Conditions To Refer

We accept referrals from pediatricians, Family Physicians or Nurse Practitioners. Please fax the required form, checklist, and documentation to the clinic as instructed below. Please follow the following steps when referring a patient to the clinic. If you are a healthcare provider and need an urgent consult, please call: 905-521-2100 ext. 76443, and ask to speak to the Pediatric Endocrinologist on service.

STEP 2: In our efforts to minimize wait times and optimize your patient’s initial visit with us, we kindly ask that you complete a disease specific checklist (below).

STEP 3: Complete the Endocrine Clinic referral form with the required information.

STEP 4: Fax both the referral form and the disease specific checklist to Fax (905) 521-5056.

Please note that we will be unable to triage your referral until the required form, checklist, and documentation are completed and attached to your referral.

Urgent Referrals: The clinics are not for emergencies. If your child needs to be seen on an urgent basis, please contact your family doctor or go to your local emergency department.
