Those who have suffered a major health event such as a heart attack, stroke, peripheral artery disease with additional risk factors such as diabetes, high blood pressure, high cholesterol, obesity, smoking, chronic kidney disease, depression are seen in the cardiometabolic clinic.
What to bring
- Your health card
- All of your current medications in their original containers, including:
- prescription medications
- vitamins
- herbal products or treatments
- over-the-counter or non-prescription medications
At every clinic appointment, the clinic clerk will ask you to show your health card and confirm your address, phone number, and family doctor.
What to expect
- If you have not had recent labs done from your primary care practitioner or since leaving the hospital, blood work and urine will be done in the 3 East lab prior to seeing the nurse practitioner.
- In person appointments will be asked to check their weight and height and vital signs to discuss their specific risk factors.
- Virtual patients are expected to get their blood work done (at life labs or Dynacare) before they are seen by the nurse practitioner and have an updated blood pressure and weight to provide for the clinic visit.
- If there are other areas of concerns you will be asked to revisit this with your family doctor or primary cardiologist. If you do not have a primary care practitioner or cardiologist please let the care team know.
