BSO/SGS Central Clinical Intake

We help connect patients living in their homes, retirement homes, and other community settings to services such as:

  • Behavioural Supports Ontario (BSO): The BSO team provides strategies and education to people with responsive behaviours.
  • Geriatric Medicine: Physicians and teams provide comprehensive assessment and care planning for older adults with age-related disorders including memory changes, falls, frailty and mobility changes.
  • Geriatric Psychiatry: Physicians and teams provide assessment and treatment planning for older adults with complex mental health disorders, addictions, behavioural changes and similar concerns.

Together, Geriatric Medicine and Geriatric Psychiatry are also called ‘Specialized Geriatric Services’, or SGS.

Our team receives the referral, searches electronic health databases to create a thorough intake summary, and matches the patient to the most suitable service. The BSO or SGS service then contacts the patient and family to book the appointment and provide care.

Our team is made up of regulated health care providers and administrative support with expertise in aging and age-related health issues as well as supports and services in our region.

Patient Information

Note that we do not call patients, families or caregivers as part of the intake process. The team relies on information from healthcare providers and electronic health databases to complete the intake assessment and match patients with the most suitable service. This means that patients and families do not have to repeat their stories again and will interact directly with the providers caring for them.

Patients and Families: If you feel that you or your family member would benefit from connection to service through the BSO/SGS Central Clinical Intake team, talk to your healthcare provider and they can make a referral.
