Meds in Hand
Retail Pharmacy at HHS has implemented a ‘Meds in Hand’ initiative so inpatients being discharged can avoid a trip to the community pharmacy in keeping with social distancing guidelines. There is no extra charge for the patient and our retail pharmacy will connect with the patient’s home pharmacy to ensure they are able to receive their refills back in their community.
As part of this initiative, we are asking that each inpatient unit to assign someone to oversee the process and ensure each patient being discharged home is informed that their medications can be made available to them at the time of discharge. Please send an email to Tina Petrelli firstname.lastname@example.org, indicating the name of your unit lead and she will provide support and guidance with this initiative remotely.
