What to Bring
- Your child’s Health Card. The Ministry of Health and Long Term Care requires us to validate your health card at every clinic visit.
- All birth records and any relevant test results or assessments from other institutions.
- Any notes or questions that you may have.
Note: If your child is ill with any antibiotic resistant organism, MRSA, a communicable disease (such as Chicken Pox), or a respiratory infection, please notify reception staff prior to your scheduled appointment.

Our team includes a plastic and reconstructive surgeon and an occupational therapist.
Resources
The Family Resource Library is located in the 3F clinic (red section, 3rd floor). Volunteer staff are available to help families find information about their child’s health or illness. Pamphlets, books, and other resources on the internet are available.
Referral Process
For Urgent referrals, contact your family doctor or pediatrician or go to your nearest Emergency Department. Do not fax urgent referrals.
Please fax new referrals to the office of Dr. Bain at 905-521-9992. Referral Form.
Referrals should be sent preferably within 2 weeks of birth, regardless of apparent severity, or as soon as possible thereafter.
We will notify the doctor once the appointment has been scheduled.
Patients are seen by the surgeon on the 2nd and 4th Friday mornings of the month. An occupational Therapist will be in touch with the family prior to begin therapy, as required.
The family will be notified if an earlier appointment is required or becomes available.
