We are committed to providing comprehensive, innovative, and collaborative family-centred care for children and teens aged 12 to 17 with a variety of health concerns. Our aim is to work with teenagers and families from diverse backgrounds with the ultimate goal of supporting them to achieve the best possible health.
The Adolescent Health Clinic works closely with other pediatric specialty clinics to provide services to youth with chronic illness and disability. Depending on the age of the teen and the reason for referral, teens are often seen alone for at least part of the assessment. Parents or guardians are always invited in for part of the consultation.

- Health card for your child – the Ministry of Health requires us to validate your health card.
- Any x-rays or letters or test results from your doctor.
- Any notes or questions that you may have.
Team Members
We have a large team with many different specialists who have a lot of experience working with teenagers. Our team includes pediatricians, a psychiatrist, psychologists, social workers, nurses, dietitians, and a physiotherapist. We also have students and doctors in training who work with us under supervision.

Referral Process
Referral Process: The referral process is made through a direct referral from a pediatrician or family doctor to a specialist. Completed referrals can be faxed to 905-521-2330. All other inquiries can be faxed to 905-521-2349. All referrals are screened for suitability.
Urgent Referrals: The Adolescent Health Clinic is not able to respond to emergencies. If your teen/child needs to be seen urgently, please contact your family doctor or go to your local emergency department.
Changing Your Appointment: Please contact us at least 48 hours/2 days ahead of your appointment should you need to change or cancel the appointment. This will allow us to book another patient waiting for services
