Medically Complex Care Services

The Medically Complex Care (MCC) unit receives patients with sub acute medical goals for short term complex medical management. The MCC team is focused on helping patients achieve their medical goals, and supporting a return home or transfer to another facility that will meet their ongoing needs.

The inpatient unit provides care for adult patients of all ages with a wide range of medical diagnoses. Our delivery of care is patient-focused and provided in partnership with patients, families, and their supports.

Care We Provide

On MCC, the goal is to provide specialized services to maintain function and progress the patient for discharge home or another setting.

Please note, Medically Complex Care is not a rehabilitation program. The primary purpose of physiotherapy (PT) and occupational therapy (OT) is to promote safety, establish a daily routine to optimize conditioning and maintain function, and assist with realistic goals towards discharge.

Patients entering the MCC program are medically stable, have a clear diagnosis/prognosis, established co-morbidities, no undetermined acute medical issues, and stable vital signs. If the level of care extends beyond what can be managed on the unit, patients will be transferred to the emergency department at an acute care site.

It is important to know that St. Peter’s Hospital is a Complex Care hospital and does not have an emergency department or intensive care unit (ICU). The discharge plan is enacted when the medical goals are resolved sufficiently for management in the community.

The expected length of stay is 30-90 days.

Referral Process

The Complex Care application can be found under the “Resources” tab on this page. Each application must include the following:

  • Submit applications when an acute level of care is not needed for a minimum of 24 hours
  • If the application is coming from a facility, please complete applications as a team to ensure that all information is captured accurately
  • Consent from the patient or substitute decision maker (SDM) is required
  • Clearly state MC goals and discharge plans

Please note that the MCC program does not offer urgent admissions, and a wait for a bed is expected if the application is accepted. In the event of a medical emergency, please go to your nearest emergency department.
