Chronic Kidney Disease/Transplant Clinic

In our Chronic Kidney Disease clinic, we see children from birth until 18 years of age with decreased kidney function (eGFR < 60 mL/min/1.73 m2) secondary to a variety of conditions (congenital and acquired) who are pre-dialysis, on peritoneal dialysis or hemodialysis, or post-renal transplant. This clinic is based on a shared-care, collaborative model, with all physicians rotating through the clinic. At each visit, families will see the multidisciplinary team including our physicians and nurses, as well as our specialized dietitian and social worker. In this clinic, we also see patients for their post-transplant care. We see about 80-100 patients over the course of a year, for a total of about 350 visits per year.

Patients/Conditions To Refer

  • Children with kidney disease and eGFR < 60 mL/min/1.73 m2
  • Children on peritoneal dialysis
  • Children with prior kidney transplant, or requiring transplant evaluation
